Summer Zoofari Camp Sign In/Out Form: Once registered, print out this form, fill in the details and bring it to the first day of camp.
Summer Zoofari Camp Request to Self Medicate Form: For campers needing to take medication, please fill out the Request to Self-Medicate form and follow the instructions on the form to send it to the Education Department.
